I've tested your form once again and was able to see the file that was attached to your form. My guess is that this was a temporary issue. However we assigned this issue to one of our developers for further investigation.
In the meantime can you please check this issue from your end? And if the issue still persists, can you give more details? (file extension, browser, e.g.)
