    How can I re-enable my form?

    I was on the website editing my forms and it now says it's no longer available

    Your account is suspended because you were phishing for credit card information, it is against our terms of use. We can activated your account if you let us delete the guilty fields.

    I don't know why my form isn't working. When I enable it says the form is disabled but when I disable if it, it says form is unavailable and that's from going through the trouble shooting videos... I don't know how it got disabled in the first place. Please help me if you can.
