I want to create a form that enable our customers to form groups. A group may contain  any number of members. Therefore I need to create a form with a button  'Add a member' that opens up inputs for name, email address etc. this is similar to the 'Add an option' feature on the radio type list.

Any ideas on how this could be done?

    Hi,

The only means in JotForm builder you can do this, is by using the Show/Hide field thru -Conditional-Logic-on-Forms .

Or, if you have means of altering your website code, then you can also use the source code embed method and modify the code to add a customize functionality such the 'add member field' you need.

Hope this help. Please inform us if you need further assistance on JotForm
