How to turn a form into a fillable PDF

I am not sure if I have understood your question correctly. Which PDF app you are trying to embed your form?

Are you trying to convert your JotForm to PDF version and unable to fill out form? I did test this by downloading PDF version of your form and I am unable to replicate this issue. I was ale to input in PDF form successfully. Please check the screenshot below:

 

The only issue is that your form do not have submit button without which your users will not be able to submit form. I would suggest you to please add a submit button in your form and then tests again.

Upon checking your form "Sign Up" has fields asking for customer's credit card number which makes it suspected of being used for phishing.

Please be noted that you should not ask credit card information from your users. If you need to process payments, please use one of our integrated payment tools. We have automated anti-phishing system which scans all forms and mark them with probability of being used for phishing activities. It's just a matter of time and our anti-phishing filter will disable your form and suspend your account.

I have deleted the credit card related fields from your form.

Asking directly for credit card data is against our Terms of Use. So, we are not able to reinstate your form to its older stance. You should use our Payment tools in order to receive payments.

trying to download 'Daily cleaning and maintenance Prossie' to a fillable pdf to use in odf expert but it isn't working.

I successfully created Fillable PDF form of your form 'Daily cleaning and maintenance Prossie'. I only noticed that submit button is not shown. Please try to remove submit button and add new one and then create another Fillable PDF form. You can also try to clone your form and then create Fillable PDF form of your cloned form. 

Also please note that you have to use Adobe Acrobat Reader or Foxit to be able to fill and submit Fillable PDF form.
