Form Stopped Showing Up

I was working on a form all day long and when I started working on a second one, no form will show up now! I am pretty frustrated as I have put a lot of work into this form! The contact form is supposed to show under Send Message on the page I provided. I am really hoping there is a solution for this!!!!!!!!!!!!

I checked your account and found it to be automatically suspended by the system. This is because of your form asking for bank account information. Is there a reason or a need for this form? 

For now, I have reactivated your account but please take note that once our filters reach your form and again finds the form asking for such information, there is high possibility of getting your account automatically suspended again. So I recommend to please remove the form or the fields in it.
