Hello,
I have checked your account and I did not see any phishing. So that, I have reactivated your account.
However, our phishing detection system was triggered because of the word "Password" that is clearly visible in your form. The form will just get disabled again if the system still detects that word. Please try to avoid to use that kind of words in your form.
Also, you can check our terms of use to avoid suspicious activities.
