I can not access my forms

I have checked you account and your forms. This form contains Password field. You can not directly get personal sensitive information such as password, credit card details. Because it violates our terms of use. If you insist on collecting any of these information, your account will be suspended again.  

I have deleted these fields and re-activated your account. Your forms are enable now.
