I get "form is not available" when I hit submit?

I did a test submissions and managed to encounter the same message.
Can you try toggling your preferences by clicking PREFERENCES at the top > under the GENERAL tab > set the STATUS to DISABLE ON DATE > SAVE CHANGES > close the PREFERENCES wizard.
Once done, do it one more time, only this time, select ENABLED.
After doing those, try doing another test submission on your form then let us know how it goes. I did those same steps on a cloned copy of your form and it worked.
