My form is reading disabled

When I click on form link, it reads Form not Found, This form is disabled.  I clicked on enable under the more section in My Forms.

Also, please avoid keep asking for these info, otherwise your account will be permanently suspended.

I have removed the word "login" from your form's title, this seems to trigger the phishing alert too, I have re-activated your account and it should be working now, please check and let us know.
