Why my forms are disabled?

    Your account has been auto suspended due to phishing by our automatic phishing detection system.

    You are not allowed to receive credit card information from users as the following screenshot of the old version of your form without using payment gateways according to our terms of use :

    As I have seen that you have already deleted the forbidden fields. So that, I have reactivated your account.
