I am not receiving form information. Can you help me?
My form has two payment choices. Obviously I need to receive the form info no matter what their choice is, but I am not receiving this info. Can you help me?

As long as your customer / visitor hits the submit button, you will receive an email notification. As i can see you are also using PayPal as a payment option. If your visitor completes the payment via PayPal, you will receive an email. Until then it will remain as an incomplete payment under your form submissions. But i have also cloned your form and made test submissions. I was able to receive email notifications. May i suggest you to change the Sender E-Mail (Votre adresse e-mail) to firstname.lastname@example.org and whitelist JotMail's Ip Addresses, if you don't receive a notification email to your recipient address?
