It seems since the new version to edit forms has been initiated (GREAT UPDATE BY THE WAY) my integration to google sheets is not working. I have gone into my PTO Request Form to verify and it seems to still be checked. would you be able to help me out?

We are currently not having any issue in regards to our Google Sheets integration. I cloned your form and I integrated it in my Google Sheets. I did a test submission and the form submission appeared in the spreadsheet.
If your spreadsheet is not updating, please try to redo the integration. It means that you need to remove the integration and then add it back.
I cleared your form cache so that your forms will be refreshed from our servers.
