How can I restore lost changes?

Hi, I am looking at my form that I saved on Sunday, and the edits are gone.  Is there any way to restore it?  Why did this happen?  I definitely don't want it to happen again.

I have checked this form http://www.jotform.us/form/63067048536156 and I can see you have applied some changes recently, here's the a screenshot about it: 

All the changes you apply to your form are recorded on the revision history tool so you could also restore those changes that you're not seeing currently in the form, here's a guide that will help you with that: How-to-view-form-revision-history

Also, changes cannot be automatically reverted back, to revert back to a previous change or undo changes it needs to be manually done through this tool, so I would also suggest you to ensure you're the only one with access to your account.
