We have your form on our website and it is not forwarding to our email this last week.... What is my account info for here if i need to make a new one?

Your email server rejected our emails because of the reason "Domain isn't in my list of allowed rcpthosts".
Please contact your email provider about the problem and they should be able to sort it.
Whitelisting the following Jotform IPs should also help.
