Please note that it is not possible to create authentication form and you should also not add form fields to asking for credit card numbers from users. Creating such form is not allowed in Jotform and it against our terms of use.

We have automated anti-phishing system which scans all forms and mark them with probability of being used for phishing activities. It's just a matter of time and our anti-phishing filter will disable your form and suspend your account.
