when people complete the form they get a check submission page which is not representative of the form or the questions and only half tyhe info is the
I checked your form but I didn't see any Autoresponder emails. The Autoresponder is the email alert that your users will receive once they completed the form. Here's a guide: https://www.jotform.com/help/26-Creating-a-Form-Autoresponder.
I checked the form notification and I can see that it is already configured properly. It means that all fields are included in the email.
Please try creating an Autoresponder email and let us know if the problem persist. Thank you.
