I have created a Form to register for a basketball league but I am not receiving the completed from to my email.
Please help,

Form that you provided has 0 submission, but it has 3 incomplete submissions. Incomplete payment usually means that someone decided to not pay upon submitting the payment form. Inside of this guide you can find how to view incomplete payments:

If you have received the payments from the user in your Paypal account but did not receive a submissions notification from JotForm, the submissions might be in the incomplete payment list. This is happening when Paypal IPN is disabled in your Paypal account. Please make sure that IPN is enabled within your PayPal account settings.
