Hello!
I just tried producing a PDF from one of our forms.
Several fields get layered one on top of the other as you see in the attached file...scratch that I can't upload a PDF.

PS Question: Why does Jotform's own form not allow a PDF upload...I find that a little concerning as PDF are uploaded all the time on the web.

Fillable PDF's are very specific when it comes to how styling is translated. Often times small adjustments make a world of difference. In your form, expanding the fields and then settings the labels to top alignment will allow for the PDF to be properly formatted:
