    This form is disabled error

    I am getting the error "This form is disabled"  when trying to view my form.  When logging in with another browser I am getting the warning my account is disabled due to phishing.  Can you please fix.

    I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I see that your form is collecting username and password which is not allowed with JotForm. It is not allowed to create login forms or collect sensitive information like passwords, Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.

    I have now removed the guilty fields from the form and activated your account. Please refrain from adding such fields to your forms to avoid any further suspensions.

    Thanks but the form is still disabled.  I was working in the form at the time you submitted this and the username/password fields were not removed when I looked.  Perhaps due to me being logged in and working in the form?

     

    I manually removed them and they are now gone.  Can you please try to activate again.  Thanks!
