My form is disabled

I have just started creating easy form to save your stats or any information that you want to save by making bookmark of "Thank you page" that will have all filled fields (At first it was made for DarkOrbit game, cause there people can make really difficult nicknames that you can't write by using your keyboard.. Like using many symbols). I made an free mail acc for that, cause it send all the time filled info, so i don't know how to disable it.. And now when im pressing "Preview" it says "This form is disabled" or smth. I dont know why, but i can't finish my "help system" now.. Can you help me? Your form builder is really good for making things like that.. so i hope you will help me.
