No matter what email address I provide in the field for the notification email field, I dont get a confirmation email informing me that someone has submitted a form. Everything worked properl up until October 31, 2016, and for some reason it no longer works. Any help on this issue would be greatly appreciated! I have done everything I can think of to fix this including changing the email address that the notification email goes to, as well as changing the send address to firstname.lastname@example.org

The email recipients associated to your form is not in the bounce list. I also cloned your form and do a test submission, it is working fine on my end.
I also check your mail logs and everything is being sent successfully.
However, I send a submission to your form and after hitting the submit button there's an error:
Please check your Thank You page redirect link if it is correct or if it is working.
Also, could you please check your email spam section? If you find JotForm emails please select all of them and mark them as not spam.
