This is usually happens when you clone a form. Here's a screen animation how to change the title of your form:
First, you can edit the title of the form in the top part of your form orange bar.
Second, open your form and click the Preferences button. Go to the Advanced tab and click the Show More and edit the Page Title. After that, click the Save Changes button when you're done.
