I must have two accounts. Please help me clear this up.

    Hello streem17,

    I did check both of your accounts and found that you seems to be using custom form URL. 

    One way to solve this problem is to move forms from your one account to another account. Please note that once the forms are moved form one account of another, the custom URL of forms moved will no longer be available.

    Do get back to us if you would like us to move form from "streem17" to "GCDSmentorship" account.
