You can undo the changes made to your form, including the removal of an element, from your form's Revision History.

To access the revision history, click the green text below your form's title.

This will bring up a menu that allows you to see all changes made to the form. Simply click on an entry made before the deletion of the elements to preview the change and click Revert if you're happy with it.
