Service Description
East Valley Placenta (hereinafter referred to as EVP) will pick up the client’s placenta(s) at the agreed upon time or will schedule a time to have the placenta(s) dropped off, depending on previously agreed upon arrangements made between EVP and said client, to be outlined in the "Client Responsibility" section below. Proper cleanliness and preparation precautions for ingesting the placenta will be carried out by EVP to ensure the highest quality of placenta encapsulation possible for the age and storage methods of the placenta(s) before the encapsulation process. EVP will deliver the finished encapsulated placenta pills in a timely fashion to the place specified by the client or the encapsulated placenta pills will be carefully packaged, and priority mailed, depending on location.
EVP will provide a detailed written explanation of the proper storing and handling of the finished placenta pills and suggestions for daily ingestion recommendations.
Limitations and Disclaimers
East Valley Placenta is not a pharmacy, pharmaceutical representative, holistic practitioner, herbalist, or medical doctor. Benefits of placentophagy are supported by ongoing research but have not been evaluated by the Food and Drug Administration. Placentophagy is a natural nutritional supplement and as such cannot be guaranteed to produce specific results.
Placenta services are not intended to replace medication or the advice of health care provider(s). Should you be diagnosed with a uterine or amniotic infection of your placenta, your placenta may not be available for consumption.
Upon receiving placenta capsules, client waive any and all rights to holding East Valley Placenta responsible for any undesired effect(s) of consuming the capsules client acknowledge that it is my personal decision to ingest or discontinue taking the capsules based on either positive benefits or any negative reactions experienced. client understand East Valley Placenta is also not liable should any other person(s) ingest my own placenta capsules or products.
I, the client, understand that East Valley Placenta will prepare, dehydrate, and encapsulate my placenta under clean conditions and adhering to OSHA bloodborne pathogen standards. The potential benefits of placenta encapsulation have not been reviewed or endorsed by the FDA and are not intended to prevent or treat any physical or mental illness.
Client Responsibility
It is the client’s responsibility to notify EVP as soon after the birth as possible (preferably the sameday as the birth itself) to request pickup of the placenta(s) by EVP at the location previously agreed upon by both parties. Failure to do so may result in delayed placenta preparation and encapsulation.
It is the client’s responsibility to discuss release of her placenta(s) after her birth from the establishment where she delivers and to ensure proper storage of her placenta(s) in a refrigerator or cooler with the placenta(s) placed on ice until it can be picked up or delivered to EVP for preparation and encapsulation.
It is the client’s responsibility to confirm with EVP they are clear from any bloodborne illness(es) or other health issues that may become infected with through contact of bodily fluids (specifically maternal and fetal blood and amniotic fluid), such as HIV, hepatitis, STD’s, etc.
Promotional Release
I, the client, give consent to Dezerrae Sanchez with East Valley Placenta to use photos taken of my placenta/placenta products in brochures, pamphlets, posters, articles, letters and/or news releases for magazines, newspapers, mailings, radio, TV, videotapes and other audio/visual media. In granting this consent, the client knows that it will be used in the best interests of East Valley Placenta and the individuals it serves as well as in my best interest. I, the client, understand my photos will not be used to identify me unless requested or otherwise discussed.
Fees
EVP Encapsulation Service includes:
• Pickup of placenta(s) to be encapsulated (if requested)
• The full placenta preparation and encapsulation process
• Proper storage container of the completed placenta pills
• Written instructions for proper storage and handling of the finished placenta pills
• Suggestions for ingestion amounts
• Delivery of the finished placenta pills to the client at the agreed upon location by both parties, or to be picked up from EVP by the client at the date and location agreed upon by both parties.
Payment includes $50 non-refundable retainer fee.
Encapsulation- $250, additional placentas in the case of multiple births include additional $25/placenta
Tincture- $25
Electronic print- $50
The balance of the total fee shall be paid in full by the time of pickup of the placenta, unless otherwise specified
Refunds
The policy of refund of payment is as follows:
• If the client decides that she does not want to go forward with encapsulation process before or after the encapsulation has been completed the client will not be refunded any moneys already paid to EVP. The client will owe EVP the balance of the fee previously agreed upon if the balance is still owed if placenta has been obtained by EVP, processing has started, or has been completed (i.e. no refund or waiving of fees will be made).
• If the placenta is not able to be encapsulated due to medical reasons a refund (minus retainer fee) will be given.
In signing this Contract, you agree that you have read and understand the Service Descriptions and Agreement for East Valley Placenta and that you agree to the terms and conditions outlined therein, as well as the stated fee.
