Unable to rollback to a previous version

Hello, yes i want revert to October 4th because I identifyed somthing problems with my form in this week. 

mmm I have a question... If I want revert another form in the future, I will need contact with you for help me with the requeriment? or could be  manually by myself?

You can directly revert to the previous version of the Form yourself. I believe you were referring to this Form:

This Form belongs to a different user and is being shared with you by him. Could you please ask the Form owner to directly revert to a previous version of his choice.

Hola, hablé con el administrador del formulario que quiero restaurar, pero cuando restauró el formulario del 4 de octubre, siempre aparece el último formulario y no aplica la reversión.
