Reporte problema de estilos en los formularios

Buenos días, estamos presentando problemas de estilos en nuestros formularios, envío imagen de evidencia. Además el widget de formulario de hoja de cálculo; ejemplo: buscador id (210466348017049) no funciona al no traer resultados, por favor esperamos una pronta solución.

Regarding the Spreadsheet to Form widget, it looks like you have uploaded a .doc file if I am not wrong.
The widget will only accept XLS, XLSX, or CSV files; it won't accept any other format.

Please try uploading an XLS, XLSX, or a CSV file to the widget.
