Thanks for reaching out to Jotform Support. You may refer to this guide How to create a survey form and customize it, you may also check our Form Templates page on available pre-made Survey forms.

Forms can be shared by getting the URL and sharing it with your users. You can share your form individually or in mass.

You may also use the feature Assign Forms. Using this feature, you can assign the form to your users by adding their email and send it to them through a notification email. However, using this feature users will have to sign in to their Jotform account in order to access the assigned form.

Thanks for reaching out to us for help. Once you've created your Survey form, you can share it using a QR code. Let me show you how:

• In Form Builder, in the orange navigation bar at the top of the page, click on the Publish tab.
• Then, click on Quick Share on the left side of the page.
• Click on the QR Code button at the bottom of the Share Form section.
• The window that opens up will have the QR code to share your form.

You can also share the URL of the form. Check out this guide to learn more about it.
