Por que la imagen que inserto en titulo en mi computadora si se ve pero en otros dispositivos no se ven

To clarify, are you perhaps referring to the image attached as logo on the heading of your form? If so, The header image and logo section are meant to display images so it will only accept image files (.jpg, .jpeg, .png). After checking your form, the file type that was used is .gif.

kindly re-upload the image again and use the file types mentioned above and see how it goes.
