    Hi,

    Your email address p****pi***q*er*taro@gmail.com is associated with the account username PROMOPIZZA. If you forgot your password, you can simply do a password reset.

    Here's how:

    1. First, navigate to www.jotform.com 

    2. When you click the 'LOG IN" button, you'll see the "Forgot Password" link or option there.

    3. After clicking that, you need to enter the email address associated with your account.

    4. You will then receive an email with instructions on how to reset your password.
