Copyright/DMCA Counter Notice

If you believe that your form or other content (“Content”) on Jotform was removed pursuant to a DMCA takedown notice by mistake or due to misidentification of the content, you may contest the removal by sending us a counter-notice at dmca@jotform.com

Your counter-notice must include all of the following six items:

1. A description of the Content that you believe was wrongfully removed.

2. The Jotform url/link where the Content was located before it was removed.

3. The following statement, if true: “I declare under penalty of perjury that I have a good-faith belief that the material at issue was either misidentified or mistakenly removed.”

4. Your name, address, and telephone number.

5. Include one of the following two statements:

   (a) If you are located within the United States: “I consent to the jurisdiction of the United States federal district court for the judicial district in which my address is located and will accept service of process from the person who provided the notice set forth above or their agent." OR

   (b) Jos sijaitset Yhdysvaltojen ulkopuolella: "Hyväksyn minkä tahansa Yhdysvaltojen liittovaltion piirioikeuden lainkäyttövallan, jossa AWS sijaitsee, ja hyväksyntiedoksiannon henkilöltä, joka on antanut edellä mainitun ilmoituksen, tai hänen edustajanlta."

6. Fyysinen tai elektroninen allekirjoitus.

Jos annat tahaltaan virheellisiä tietoja ilmoituksessa, tämä saattaa johtaa rangaistuksiin tai sakkoihin.

Me emme tarkasta lähettämiäsi liiteitä.

Tarkistamme vastailmoituksesi saatuamme sen. Jos uskomme sen olevan täydellinen ja pätevä, välitämme antamasi tiedot edelleen sisällönpoistopyynnön jättäneelle henkilölle. Jos he eivät ilmoita meille 10–14 arkipäivän kuluessa, että he ovat nostaneet kanteen sinua vastaan, ryhdymme toimenpiteisiin poistamamme sisällön palauttamiseksi.
