    INSTRUCTIONS FOR APPLICANTS

    To start, click on the title bar "Page 1" below. The section will open revealing a set of questions. (Hint: click anywhere in the top half of each title bar to open or close it.)

    Required fields have a red asterisk (*). You will not be able to submit the form until these questions have been answered.

    Once you are done with a section, move on to the next section by clicking on its header. The previous section will close and the new section will open. You can go back to a previous section by clicking its header bar.

    • Research Proposal

      Attach a concise description of the proposed research or project, not to exceed ten (10) double-spaced pages, which include the following:

      • A brief review of background research/literature, including any preliminary work by the applicant, specific to the proposed research question or project (not to exceed one page).
      • A clear statement of the purpose of the project.
      • A description of the methodology to be used, including the population to be studied; justification for test instruments selected and data analysis techniques proposed, in the case of research; and the proposed time frames for various components of the project.
      • Implications for practice.

      Attach letters of approval from the autorizing institution department head/facility dictor.

      These must include:

      • verification of adequate time, space and resources, such as secretarial help to carry out the study
      • waviers of administrative overhread and grant administration fees
