the concept and creative execution of the motion picture for Academy Award purposes if not

only the authorized person with the right to the movie should or must delegate authorization

have read the attached rules governing the Golden movie Awards submission for short film/

movie and accept them.

• I certify that all the information provided on this Entry Form is correct.

• I understand that according to Academy rules, each film print or copy submitted to the

Academy will be held until the voting process is completed and then, if the motion picture

is not nominated, will be returned to the filmmaker. I further understand that if the picture

receives a nomination, the Academy will retain a print for its archives, and my permission

is hereby given for the Academy to select, tape, transfer, and broadcast an excerpt from

the picture on the annual Awards presentation.

• I agree to allow the Academy to stream my film for voting purposes. I agree that all

questions of eligibility and rules interpretation will be resolved by the appropriate

Academy committee.
