I hereby designate this form as notification of check-out of my current housing assignment of Spring 2020.
By designation, this form will serve and represent the resident and the damages of the room/apartment with the same effect as if the Resident were physically present for inspection and I understand that I will be billed for any damages/cleaning recorded within my housing assignment.
Completion of this form will serve as official notification of check out of my Spring 2020 housing assignment.
Area Coordinator staff will walk through all residence halls and apartments to assess for damage and cleaning fines by April 10th.
I understand that I need to drop my key off at the Residence Life Office or at the Bubble prior to leaving campus. Failure to drop off room key, will result in a key replacement fine. Any key not received by March 30th will be considered lost.
