  • Coronavirus Questionnaire

  • Have you or anyone you live with travelled outside the UK in the last two weeks?*
  • Have you been unwell, had a new cough, temperature or headache?*
  • Has anyone in your household being diagnosed with coronavirus or had coronavirus type symptoms in the last 14 days?*

  • If you ticked yes in any of the above boxes, then please call us to discuss and rearrange your appointment - you will not be charged.  

    If you ticked no, then please:

    • arrive exactly at your appointment time and not earlier to avoid mixing with other patients.
    • arrive without relatives/ visitors. 
    • bring a large towel for the bed so you avoid touching the linen/ pillow.
    • arrive wearing a mask. (If you don't have one, we will provide one.)
    • dispose of gloves (if you are wearing any) on arrival in our bin and use alcohol gel.
    • allow the clinician to take your temperature using an infrared device. 

    NB - the Clinician will be wearing a mask and a plastic apron.  Their hands will be disinfected using an alcohol gel.  All surfaces will be disinfected prior to and after your appointment in the 15 minute gap between patients.
