2020 End of Year Programming Assessment, Committee Selection, and Service Hours Survey
This year the programming committee worked hard to ensure our programming was HELPFUL to the Community&ENGAGING for the children/teens&APPLICABLE in their everyday lives&ROOT in Jack and Jill TRADITION. Programming is very much like taking care of a garden. Often times you have to weed your garden and nurture your crop. For us this nurturing includes effective planning, proper coaching/grooming of age group sponsors and teens, and periodic programming assessment. Please complete this survey to assist with improving our programming to meet the needs of our chapter. For the Children!!!

In an effort to collect the number of service hours families in our chapter have earned, we need the information below from each family. Please complete each question/statement as requested.
