1.The undersigned (“Adopter”) acknowledge and agree that the Rescue Partner has completed a thorough interview with Adopter via a live feed video format (such as Facebook Messenger Video, Whats App, FaceTime or Skype) (the “Interview”). a.During the Interview, the Rescue Partner thoroughly explained the process of an out of state adoption through the Rescue Partner and Project Freedom Ride (“PFR”) (including the procedures that Adopter is required to follow if the adoption is unsuccessful, which procedures are set forth in Section 4 below). b.During the Interview, it was made clear that dogs, much like children, require guidance and although the Rescue Partner does their best to set all their pets up for success with their families, enlisting the help of reputable trainers is strongly encouraged when needed. c.Within the Interview, the Rescue Partner thoroughly explained the measures that Adopter should take when welcoming a new pet into our home, including, but not limited to: i.Decompression time; ii.Introduction steps for the new addition with current pets within the home; iii.Not overwhelming the dog with dog parks, house guests, etc within the first month; iv.Establishing care with Adopter’s local vet within 60 days of arrival ; and v.Verifying Microchip has correct contact information and remains up to date throughout the life of the pet *

2.If adopting a working or herding breed: a.Adopter is aware that these dogs typically need more guidance, regiment, and purpose and without such can exhibit destructive, undesired behaviors. Adopter will contact the rescue partner immediately and seek help from a reputable trainer should concerns arise; b.Adopter is aware that herding breeds tend to be more mouthy and nippy and have a tendency to exhibit herding behaviors with other animals and potentially humans if not properly trained; and c.Adopter is aware that herding breeds can be more prone to a strong prey drive. *

3.Adopter acknowledges and agrees that any and all fees paid by Adopter in connection with the adoption contemplated hereby, including the adoption fee and any transport fees, are NON-REFUNDABLE.*

4.Adopter acknowledges and agrees that if, for any reason, the Adopter is unable or unwilling to keep the pet and provide the pet with the care necessary or advisable for its health and well-being, the Adopter shall immediately contact both PFR and the Rescue Partner to return the pet to the Rescue Partner, at the sole cost and expense of the Adopter, before making any other arrangement with respect to the housing of the pet. The Rescue Partner and/or PFR will notify the Adopter if either of them has determined that it is able to take possession and/or secure alternative placement of the pet. Before such time as PFR and/or the Rescue Partner takes possession or places the pet with an alternative home, the Adopter agrees to maintain the health and well-being of the pet, either in the Adopter’s home or at a safe and professional boarding location. The Adopter agrees that it shall not sell, move, gift, or otherwise transfer the pet to any other person (including any family member or shelter) without the prior written consent of PFR or the Rescue Partner.*

5.Adopter acknowledges and agrees that they have had an opportunity to ask questions regarding the adoption contemplated hereby. Adopter acknowledges and agrees that none of PFR or any PFR Related Party (as defined below) has made any representations or warranties regarding the subject pet, the adoption contemplated hereby, or any related matter, and Adopter is not entitled to rely on any representations and warranties of PFR or any PFR Related Party with respect to such subject matter. Adopter, on behalf of itself, its successors, heirs and assigns, hereby generally, irrevocably, unconditionally and completely releases and forever discharges PFR, its affiliates, and its and their directors, officers, managers, employees, agents and advisors, and each of their respective successors and assigns (collectively, “PFR Related Parties”) from all disputes, claims, losses, controversies, demands, rights, liabilities, actions and causes of action of every kind and nature, whether known or unknown, arising from any matter concerning the subject pet, the adoption contemplated hereby or any other related matter or agreement between Adopter and the Rescue Partner and/or PFR, or any matter contemplated by or related thereto.*
