Thank you for your time. We are sharing with you this informed consent in order for you to understand the purpose of our study and why we are inviting you to participate in this research. Please take your time on reading this informed consent and if you have questions, please inform us so we can answer them accordingly. The information provided in this form shall be emailed to you, together with the information you have provided to us. Please take note that this is purely voluntary and you may withdraw from participating before, during, or after your participation.

We have randomly selected 500 people for this study who are frequently online for various reasons. Others might be due to social media, others for work. In any case, we would like to understand the natural human behavior of the most common users of the internet.

There are no known risks when taking part in this study. However, there might be questions that might be uncomfortable to answer. If you decide to participate and encounter questions that may be difficult for you to answer, you can let us know.

In case an accident occurs when you are on-site or with one of our representatives on the field taking part in our study, please take note that we do not have a paramedic on stand by. If an emergency happens, we can call 911 immediately or assist you to go to the nearest hospital. In case you are somewhere remote and out of our reach, you should call 911 right away or go to the nearest hospital.

After signing this informed consent, while taking part in the study, you may encounter questions that you might feel uncomfortable to answer and/or you may get to know more and understand more concepts about this study.

The submitted forms shall be categorically grouped. The information submitted shall be stored in a secure database where only authorized persons are allowed to have access. You may get a copy of the result of your result. Just let us know and we will submit it to you via email.

This study will also be published on the website. All information and data gathered shall appear there, but no personal information that can identify you shall be disclosed.

There are no fees to pay for taking part in this study. For participants, we will give a $10 food voucher that can be used on major fast-food establishments.

Feel free to ask us questions or raise with us some concerns, any time. It is your right.

By signing this form, I hereby declare that I have read this electronic informed consent, or the contents have been read to me. I have had the time and opportunity to ask questions to the facilitators and researchers of this study and my questions were answered to my satisfaction.

I understand that my participation in this study is purely voluntary and I can withdraw my participation anytime. I also understand that should I decide to withdraw, the information I have provided will permanently be deleted from the records of the researchers and/or sponsors, without my relationship with them being affected.

I have read the information sheet to the participant, accurately and made sure the participant understands, to the best of my ability. I confirm that the participant had the opportunity to ask questions, and I confirm that the participant has freely given his/her consent to this study.
