I grant Haverhill Community Care full rights to use the images resulting from the photography for fundraising, publicity or other purposes to help achieve the company's aims. This might include (but is not limited to), the right to use them in their printed and online publicity, social media, press releases and funding applications.
I give Haverhill Community Care permission to use these images in my care and support plan and/or my 'All about me' page.
I also give Haverhill Community Care permission to take photographs for health, medical and well-being reasons.
