I understand that I have entered into this premise because I am opting for a service that is not urgent and not medically necessary.

I understand that J & K Nails have put in place preventive safety measures for the benefit of myself, other clients and the staff. I understand this includes the closure of toilet facilities for clients.

To prevent the spread of contagious viruses and help to protect others, I understand that I will have to follow the salon's strict guilde lines, including the following:

• Wearing a face mask before, during and after my appointment
• Washing my hands before and after treatment
• Keeping social distance measures provided by the Government
• Coming to my appointment alone and only at my appointment time

I also understand that I may contract COVID-19 from anyone within or outside this premise without knowing it. I, therefore, will not hold J & K Nails and their staff members offering the service responsible for any liability related to COVID-19 and any variation or mutation thereof.

I have been given the option to defer my service to a later date, however, I understand all the risks including those noted herein and I would like to proceed with this service.
