If all requested documentation is not provided the application will not accepted or reviewed.
Please provide thefollowing information:
Proof of all household income for thelast two months. (Paystubs,Unemployment Compensation Benefits, Social SecurityBenefits Letter, Public Assistance Benefits Letter, Etc.)Social Security Cardsand ID for yourself and all household members.Provide Proof BirthDates of all the household members.Letter ofcircumstance (Reason you are requesting assistance)Proof of Home Ownership or Deed ofProperty.
