- This should be completed 1-2hrs before your training sessions
- You must complete a new questionnaire for every training session.
- If you answer yes to any of the question, you cannot attend training.
You will receive an email response with your submission, if you do not receive this email confirmation then we are unlikely to have received your response.

Have you or anyone in your household been in contact with someone with COVID-19 in the last 14days?
Have you or anyone in your household been overseas on the quantise list or exposed to someone with Covid-19in the last 14 days?
Have you or anyone in your household had COVID-19 symptoms (self-isolate for 7 days) or are feeling unwell? > a high temperature or fever > a new continuous cough > a loss of, or change in sense of smell or taste
Have your or anyone in your household been told to self-isolate as part of the contact tracing (Test & Protect) measures in place?

If you have coronavirus symptoms
If you’ve developed symptoms (however mild) in the last 7 days, stay at home for 7 days from the start of your symptoms and arrange to be tested. Do not go to your GP, pharmacy or hospital.
