This form is to be completed when a family has a physical address change.* This multipurpose form can be used to submit a proof of residency document or complete an affidavit of residence.
Only one form is required for each household as you can enter all enrolled students on this document. You do not need to complete this form for each student. If you have questions regarding this form, please reach out to your Teacher or the Proof of Residency team at firstname.lastname@example.org.
*Mailing address changes can be emailed directly to your Teacher, completion of this form is not necessary for that purpose.
This form is to be used either when the parent/legal guardian is currently residing with someone and cannot provide legal proof of residency for themselves or to assist in determining if the student meets the definition of a child in transition (No permanent nighttime residence). This affidavit is intended to address requirements of the McKinney-Vento, Title X, Part C of the Elementary and Secondary Education Act.
Parent(s) or guardian(s) will need to complete a “Caregiver’s Authorization Affidavit” in the event the child is not staying with his/her parent(s) or guardian(s). If you are a parent or guardian and require that form, please reach out to your Homeschool Teacher for the document.
