    Self-report Measures - General
  • This form contains two self-report measures which ask about your thoughts and feelings, and how your mental health condition affects your day-to-day life. You will need to complete this form before you start your therapy and again at the end of your therapy so that you and your therapist can identify any changes to your well-being after you have completed your therapy. 

     

    The Psychological Therapies Hub also uses information that you provide using this self-report measure to evaluate the effectiveness of the intervention and for audit and research purposes. For these purposes, your infomation will be annonymised.

  • People's problems sometimes affect their ability to do certain day-to-day taks in their lives. To rate your problems, look at each section and determine on the scales provided how much your problem impairs your ability to carry out the activity.
