ABOUT THE STUDY
This is a research project that shall serve as partial fulfillment of the project of John Smith and his group for his class. This survey aims to identify the best teaching practices practiced by teachers and in order to understand the perception of students.
WHY ME?
You are being asked to participate in this study because you are a student or a teacher.
PARTICIPATION
Please understand that your participation in this survey is voluntary and you may, anytime, refuse to take part in this research. In case you do wish to withdraw, please let anyone from John Smith's group know your wish to withdraw. You may freely decline in answering particular questions that may feel uncomfortable to you.
BENEFITS
You will not gain any material benefit in your participation in this research. However, you will feel good in helping us reach our intention in completing this project and may lead to further progress in how the University will address any difficulties or challenges that may come out on this research.
COSTS
Your participation in this project will not cost you anything.
RISKS
There are no known foreseeable risks involved in participating in this study other than those encountered in day-to-day life. However, there might be some questions that may give you discomfort in answering. In this case, you may let us know that you intend to not answer the specific question involved.
CONFIDENTIALITY
The answers to the survey will be stored in JotForm.com. Access to the information is password protected and secured. We will not share your name, IP address, or any information shall personally identify you. We assure you that any information we receive will be held in trust and confidence.

UPDATES/CHANGES IN THE STUDY
We will reach out to you if you leave your contact information with us and if you are willing that we keep you posted, any changes or updates in the furtherance of the study will make us update there are significant changes in the study that may appear that you would be interested in.
