The Youth Connection Career Academies Program

Each participant AND parent (if under the age of 18) must read and sign the following written agreement:

General Participation

1. I understand that I will be terminated from the program, and may be subject to legal action for falsifying information about my eligibility for the program including: age, family income, Detroit residency, citizenship, and social security number.
2. I understand that after completing the orientation and registration process, I will complete a thorough intake process to ensure that TYC can identify all or specific program components that best align with my needs, to help me develop my career and educational goals and identify any supportive services needed, if any. I further understand this may require TYC to request additional information from me and/or my parent/legal guardian to complete my program enrollment.
3. I understand that within 30 days of enrollment and no later than June 15th each year, I must submit a health appraisal/physical that was completed after September 1st of the prior year. This form will only be valid for 12 months from the date the exam was completed. I will be required to complete a new exam upon expiration to remain enrolled in the program. TYC will provide me with a copy of the health form to be submitted. I understand I am permitted to submit a copy of the form provided by my doctor if it provides the same information as requested on the TYC health form. By signing this agreement, I agree and understand that I am authorizing TYC to transmit the health appraisal form to the appropriate personnel and program affiliates. I understand I will not be permitted to attend any field trips, outings, or report to a worksite until a current exam and form have been completed and submitted to TYC staff.
4. I agree to participate in The Youth Connection Career Academies (TYCCA) program activities, including but not limited to: participant orientation, various training workshops, forums, work experience, camping, canoeing, fishing, low/high ropes and other related outdoor program activities, outings, field and focus trips.
5. Attendance and punctuality are very important! I understand that I am only allowed three (3) unexcused absences per quarter. I also understand that three (3) unexcused late arrivals equal one absence. I further understand that poor attendance, unexcused absences, or excessive tardiness will result in a reduction in my stipend/paycheckor incentives, including and up to my dismissal from the program. If I must be absent or late, I agree to call my supervisor, instructor or other appropriate TYC staff in advance.
6. I agree to participate in all monthly scheduled program activities, workshops/trainings and community service in order to be eligible for any program incentives, stipends/payand the paid summer internship. Hours and locations may vary. I will be provided with a program schedule and it is my responsibility to retain a copy as I will be expected to follow it accordingly.I understand that if I regularly demonstrate poor attendance, TYC staff has the discretion to terminate me from the program at any time and I may not receive any further payments.I also understand that if I am dually enrolled in a training or certification program at a partner location, I am still required to attend program activities at The Youth Connection to remain in good standing with the program.
7. I understand the training workshops may include, but arenot limited to,various career fields,topics related to outdoor education, selfesteem, selfimage, death/family loss, sexual education and other health related topics, black history, current events, including photographs which may be graphic in nature and other topics at the discretion of TYC staff. I further understand that every precaution and care will be taken to ensure age appropriate, accurate and current information is provided.

8. I understand that I am eligible to be placed in a paid internship during the summer months. However, in the event I have not completed the necessary program components (i.e. late enrollment, absences, etc or if I am not selected by a worksite provider after completing an interview, I understand that I may be assigned to complete additional training in place of being placed at a program worksite. I further understand this is totally at the discretion of TYC staff and I will be notified of their decision.

9. If my assigned worksite is closed temporarily or permanently, I understand that an attempt will be made to find an alternate worksite for me, but there is no guarantee. If I cannot work, I understand that I cannot receive a stipend/paycheck.

10. I agree and understand that the program activities schedule and the summer employment worksite placement assigned to me cannot be changed, unless unforeseen circumstances are in effect.

11. I understand that unacceptable conduct at The Youth Connection or at a partner training or worksite will cause me to be sent home, lose all or part of my stipend/paycheck or incentives and may result in dismissal from the  program. Unacceptable conduct includes, but is not limited to: using profanity or other derogatory/demeaning language, fighting, illegal activity; theft, destruction of property, insubordination; excessive horse playing or goofing around, or inappropriate sexual conduct. 12. I understand that once I arrive to TYC for any program activities, I am not allowed to leave the premises for any reason until I am dismissed by TYC staff. I also understand I am not allowed to leave my assigned training site or worksite, for any reason, without permission from my supervisor, if I am over 18. If I am under 18, I understand I am not allowed to leave my worksite without permission from my supervisor AND parent. I further understand I must obtain permission in writing from my parent, in advance. This includes leaving for meals and snacks.

13. I understand I am expected to wear appropriate attire and be groomed daily. If I do not follow this rule, I may be sent home. Complete dress code requirements are discussed during orientation.

14. I agree to display honesty and integrity at all times. I will respect all The outh onnection (TYC) staff, chaperones, training partners, other participants and assigned supervisors, regardless of my or their age, race or gender. I further agree to listen and follow all rules and regulations of TYCCA Program and the rules and regulations of my assigned worksite(s).

IN THE YOUTH CONNECTION CAREER ACADEMIES PROGRAM. I further understand, I will be provided with a detailed participant handbook outlining all program guidelines and will be expected to acknowledge receipt in

writing as requested by TYC. I understand that TYC operates multiple programs and receives various funding sources. Therefore, program information and guidelines that apply to me may not necessarily apply to other participantsand I should govern myself according to the information provided to me by TYC staff ONLY.

15. I understand that I will only receive a stipend/paycheck or incentives for the authorized time as shown on my TYC sign-in sheet or timesheet with my signature, and that all timesheets must be signed by a supervisor or they will not be processed. I also understand that if I falsify any information on my sign-in sheet or timesheet I will be terminated from the program.

16. I understand that when I report to an assigned training site or worksite, I am expected to complete my training, work my entire shift, and complete all tasks safely as assigned by my supervisor or trainer. Failure to do so will result in a reduction in my stipend/paycheck and/or termination from the program.

17. I understand that I must sign-in when I arrive to the program/training site or worksite and sign-out when I leave. No one else may sign-me in or sign-me out. I must follow all timesheet procedures. Failure to follow these steps will result in a reduction in my stipend/paycheck and/or termination from the program.

I HAVE READ THE PARTICIPANT AGREEMENT AND UNDERSTAND THE CONDITIONS OF MY PARTICIPATION IN THE YOUTH CONNECTION CAREER ACADEMIES PROGRAM. I further understand, I will be provided with a detailed participant handbook outlining all program guidelines and will be expected to acknowledge receipt in writing as requested by TYC. I understand that TYC operates multiple programs and receives various funding sources. Therefore, program information and guidelines that apply to me may not necessarily apply to other participants and I should govern myself according to the information provided to me by TYC staff ONLY.
