    This form is for those who have tested positive for COVID-19 and meet the requirements below for removal from isolation.
  • You can return to normal activities when:

    If you are sick:

    1. At least 24 hours after you are free of fever without the use of fever-reducing medications and other symptoms have improved, AND
    2. At least 10 days after the date you first had symptoms.

    If you do not have symptoms, you should remain in isolation for:

    1. At least 10 days after your test date.

  • You do not currently meet the requirements to be released from isolation.  Please continue to stay home until:

    1. You are free of fever without the use of fever-reducing medications and other symptoms have improved, AND
    2. At least 10 days after the date you first had symptoms.

    • At least 10 days after your test date.
