- The employee must provide accurate information in this form.
- If your application is approved, the allowance will be credited to your preferred bank account. You may choose to be credited via wire transfer or PayPal.
- The accounting department will ask you to upload or email the receipts of your purchase for reimbursement. Or you can select an online store and the appropriate team can order it and deliver it to you.
- Please take note that "Others" expenses might not be covered. Kindly get in touch with your HR department to see the coverage.
- If you leave the company for less than 6 months, you need to return the work-related equipment that was paid by the company.
- If you stayed in the company for 1 year or more, this equipment will be yours and we will send you a different agreement.
- If the equipment needs repair, the employee will be responsible for sending it to the store for repair.
- The employee must take extra care for the equipment and maintain it well.
