In consideration for providing permission to Participant by the Company for participation to the above-mentioned event, and being permitted to access the facilities of the venue, the Participant agrees as follows:

Assumption of Risk

Participant acknowledges that inherent risks are involved in the Event, that may result to injuries, illness, including the exposure to COVID-19 which may lead to complications that affect the respiratory system, kidney failure, and other damages to the organs that can lead to or cause death. Participant likewise understands that although the facility has set some measures in place, accessing the facilities bear risk of exposure where anyone including Participant may contract COVID-19 and may transmit to others. By entering into the premises, Participant assumes all the risk and dangers that could arise on the forgoing during the Participant's participation in the Event and assumes all risk and take full responsibility.

Compliance and Participation to Risk Mitigation

Participant agrees to comply in the process of reducing any risk of exposure, infection, and/or transmission of COVID-19 during his/her participation in the event by doing the following:

1. Undergo the process of screening and evaluation such as, but not limited to, answering truthfully a COVID-19 risk assessment evaluation, temperature checking.
2. Maintaining sanitation and following the recommended practices, guidelines, and policies at all times, including, but not limited to social distancing, wearing of face masks, and keeping hands sanitized at all times.
3. Notify the administration, health, or organizing committee should the Participant starts to feel or exhibit symptoms of COVID-19, such as, but not limited to shortness of breath, fatigue, muscle and/or joint pains, loss of taste, sore throat, diarrhea, nausea, or fatigue anytime from 14-days prior, during, or after entry to the facility.
4. Notify the administration, health, or organizing committee whether Participant traveled to areas with high-risk exposure, or an area currently subject to a CDC Level 3 Travel Notice 14-days prior to my entry to the facilities.
5. Notify the administration, health, or organizing committee whether Participant has been advised to self-quarantine by a healthcare professional due to possible exposure or infection to COVID-19
6. Submit to tests conducted by the administration, health, or organizing committee or are currently waiting for results from tests for COVID-19.

Release and Waiver of liability

The Participant hereby release, discharge, waive, covenant not to sue, and hold harmless, the Company, its employees, affiliates, directors, and officers, from any liability resulting from accident, illness, physical injuries, or death, arising from or in relation to his or her participation to the event.

Indemnification

Participant hereby agrees to indemnify, defend, and hold harmless the Company, its employees, affiliates, directors, and officers, including its assigns, from any and all claims, suits, costs, damages, or liabilities, including, but not limited to attorney's fees arising from, or in relation to Participant's participation on the event.

Governing Law

This Agreement shall be exclusively governed by the laws of the State of {state7} without regard to principles of conflict of laws. 

Separability

Should any of the provisions or clauses in this Agreement be held invalid by a competent court, such invalidity shall affect only the said provision and the remaining provisions shall remain valid and enforceable.
