Please fill out this Continuing Enrollment Form and submit the form before October 10, 2020. Enrollments submitted after October 10 shall constitute a late enrollment fee of $25.00. After which, the last day to submit Continuing Enrollment shall for Fall be on October 31, and for Spring, on March 31.
